ADDITIONAL INFORMATION

  • Labs and other Tests:
    In order to provide optimal treatment, there may be a medical necessity for lab and or other tests. It is your responsibility, as the patient, to obtain the requested examinations. Failure to complete these tests within a reasonable time may result in delay with refilling active medication or prescribing certain medications. Urine drug screenings, as deemed necessary, are conducted on patients. Initial drug screening is mandatory for those prescribed controlled medications, and random screenings throughout treatment. The patient is responsible for any charges associated with urine drug screenings not covered by the insurance company.
  • Dismissal From Practice:
    Hardly occurs however, a patient may face dismissal or discharge from the practice. Common reasons for dismissal include consistent appointment cancellation, verbal or physical abuse towards staff, failure to pay outstanding balance, and treatment non-compliance. Once discharged, a notification letter will be sent to your listed address to inform you of the dismissal or discharge, and 30-day refills of medications provided. After discharge from the practice, patient will be unable to schedule additional appointments or request more refills.

Our Mission

Tranquil Pathway Mental Wellness, is guided by a mission rooted in understanding, empathy, and individualized care. Inspired by the struggles individuals face due to stigma and misunderstanding, our aim is to dismantle barriers and offer compassionate care, guiding individuals toward healing and wellness.

Confidentiality and HIPAA: Patient information within the electronic medical chart is accessible to trained office staff for scheduling and medication refills. Patient information will be available to the collaborating physician. However, without written consent or a court order, no information will be released to other parties.
 
There are several circumstances where confidentiality cannot be guaranteed, such as:
- If the patient is a danger to themselves or others, or a case of child or elderly abuse/neglect (we are mandatory reporters)
- Court-ordered release of information
- Malpractice lawsuit or billing dispute
- Request by your insurance company for case review
- Credit card statements will show the name of the practice
- Unpaid balances sent to a collection’s agency or other responsible party if bills remain unpaid
- Information exchange within the practice among administrative staff, colleagues, or for professional consultation

Practice Process pertaining to medications


Medication Refills: As the patient, it is your responsibility to inform your provider during scheduled appointments if you require a medication refill. The provider maintains the right to decline refill requests in instances of missed appointments or frequent cancellations. Such actions could result in dismissal from the practice. Refills will not be issued after hours or on weekends. Please notify the provider before running out of medications and allow up to 3 business days for the refill process.
 

Controlled Medications: Per legal requirements, an in-person appointment is necessary for the prescription of controlled substances. No prescriptions for controlled substances will be issued without an initial in-person consultation.
 

Controlled Substances (e.g., stimulants such as Adderall, Vyvanse, Focalin, Benzodiazepines such as Xanax, Klonopin) can only be prescribed in intervals of no more than three months. Appointments must be attended to receive refills due to the strict regulation of these medications. If a controlled medication is lost, it will not be rewritten until the next scheduled refill time. Patients on controlled medications undergo random urine drug screenings while on controlled medications. The provider reserves the right to refuse medication refills and/or dismiss a patient from the practice if urine drug screenings are not completed as requested.
